By filling out this form, you are choosing to
pay month by month.  You will receive an invoice on the first of every month from September - May.  
Please fill out the form based on 
total amount of hours  for your entire family.
Please note:
Jubilee registration is the for the entire year.  
Tuition fees do NOT include recital fees, costume fees or any other additional fees that may arise throughout the year.
